Pride Scholarship Form - Must be turned in with Contract Agreement if seeking a scholarship
2024 Permission to Treat - Google Form - Must be completed by May 15
PHYSICAL FORMS
All students in the Bentonville Band Program are REQUIRED by the Arkansas Athletics Association (AAA) to have a physical completed by July 1. You can scan this form and send to Mr. Hendrix at thendrix@bentonvillek12.org please make the subject line: Physical Form - Student Last Name
AAA Physical Form - Must be turned in by July 1 with Physician Signature
AAA Sports Physical History Form - This Form will stay with your doctor and placed in your medical records.
IMPORTANT FORMS
Overnight Medication Form - This form must accompany all needed medicine for your student, including over the counter medication.
Alternate Travel Form - This form allows a non-family member to transport your child from an event. The form must be turned in 5 days before travel for prinicipal signature and approval.
INSTRUMENT RENTAL/REQUIRED INSTRUMENT LIST
Preferred Instrument/Required Mouthpiece - Student must have before first day of Summer Band Camp
Required Percussion Stick/Mallet List - Student must have before first day of School.

Practice-A-Thon - Kicks off August 1st
Worlds Finest Chocolate - Students must complete form to sell chocolate.
