PHYSICAL FORMS
All students in the Bentonville Band Program are REQUIRED by the Arkansas Athletics Association (AAA) to have a physical completed by July 1. You can scan this form and send to Mr. Hendrix at thendrix@bentonvillek12.org please make the subject line: Physical Form - Student Last Name
AAA Physical Form - Must be turned in by July 1 with Physician Signature
AAA Sports Physical History Form - This Form will stay with your doctor and placed in your medical records.

EXEMPTION FORM
Marching Band Exemption Form - If applying for Exemption as stated in the Handbook. This form must be completed by May 15 (No later than June 1) and returned to Mr. Hendrix (thendrix@bentonvillek12.org).
